Product klacht

Gelieve dit formulier enkel te gebruiken voor het melden van een kwaliteitsdefect aan één van onze producten. Gelieve één klachtenformulier per batchnummer te gebruiken. Indien mogelijk graag een foto doorsturen van het kwaliteitsdefect. Graag het klachtenstaal bewaren tot nader bericht.

Alle verplichte velden dienen ingevuld te worden om uw productklacht te kunnen verzenden. Onze kwaliteitsafdeling zal met u contact opnemen voor de praktische afhandeling van uw klacht en het eventueel transport van de klachtenstalen.

Condition of the complaint samples:
Sample possibly infectious (blood)

In case the sample is possibly infectious the neg.serological attest is needed for sample pick-up
