Plainte relative à un produit

Veuillez utiliser ce formulaire uniquement pour signaler un défaut de qualité concernant l’un de nos produits. Un formulaire distinct doit être rempli pour chaque numéro de lot. Si possible, transmettez une photo illustrant le défaut constaté. Veuillez conserver l’échantillon concerné jusqu’à nouvel ordre.

Tous les champs obligatoires doivent être complétés pour pouvoir soumettre votre réclamation. Notre service qualité prendra contact avec vous afin d’organiser le traitement pratique de votre plainte, ainsi que le transport éventuel des échantillons.

In case the sample is possibly infectious the neg.serological attest is needed for sample pick-up
